Through your PennChart account, you can access your Student Health and Counseling records and upload and manage your immunization records. Please note: you will be directed to your MyChart through myPennMedicine.
If you don't have an existing myPennMedicine account, you must create and activate your account by clicking the button below. After you fill out this form, you will receive an email from our team with next steps within 2 business days. If you have questions, you can reach out to our Immunization Navigators by calling 215-746-4200 or emailing wel-immun@pobox.upenn.edu. You can also visit them in person at 3535 Market Street, Mezzanine, Suite 60.
